There’s no place like home. Sometimes, after a hospital stay, you need extra care before you are ready to return home. Swing Bed care is a specialty service provided only by select, licensed hospitals. The program provides medically supervised care and is specifically designed to fill the gap between your hospital stay and going home.
During your Swing Bed stay, hospital staff and medical staff will work together, with you, to create a treatment plan and health goals specific to your needs. Patients can participate in their own care and continue their recovery before ever leaving the hospital. Most importantly, our focus is improving your physical, nutritional, and medical condition so you can return home, where you want to be.

Services We Provide
Most Swing Bed care patients require daily care, provided by a registered nurse and/or licensed therapist. Our caring, qualified staff provide many services including:
- 24-Hour Skilled Nursing Care
- Nutritional Services Provided by a Registered Dietitian
- Case Management & Social Services
- Pharmacy & Laboratory Services
- Discharge Planning
- Rehabilitation
- Additional therapies for recovery such as physical, occupational, or speech
- Intravenous therapy and/or medications
- Wound care
- Medication management
- Patient/Family Training & Education for self-care such as:
- Self-catheterization
- New ostomy
- Administration of injection medications
- Feeding tube care
- Management of PICC line
Comprehensive Care
Patients receive skilled evaluation, rehabilitation, and treatment services from our care team:
- Transitional Care Physician
- Care Coordinators ensure a smooth transition to the Swing Bed Program and will be involved throughout the duration of your stay
- Our Rehabilitation Team include physical therapists, occupational therapists, speech therapists, and other rehabilitation services as needed
- An on-site Registered Dietitian determines your nutritional needs, tracks your progress, makes dietary recommendations, and provides education
- Pharmacist
- On-site Radiology and Laboratory team

Swing Bed Program Eligibility
Our Care Coordinators will work with you to determine your eligibility and financial responsibility. Patients are evaluated and admitted to the Swing Bed Program based on their medical needs. Once it is determined that Swing Bed care is appropriate, to be admitted the patient must:
- Be accepted by a physician on our medical staff to oversee the care
- Need skilled nursing care or therapy for a short-term period
- Have a 3-day, qualifying inpatient stay during the previous 30 days for the same illness
- Be willing and able to actively participate in therapy and the plan of care
